Patiënten met de ziekte van Parkinson en dementie komen nu niet in aanmerking voor behandeling met diepe hersenstimulatie (DBS). Het is echter de vraag of het gerechtvaardigd is om deze patiënten een potentieel effectieve behandeling van motorische responsfluctuaties te onthouden. Uit de literatuur blijkt dat DBS geen invloed heeft op de cognitieve achteruitgang bij parkinsonpatiënten met normale cognitie en met geringe cognitieve stoornissen. Tevens is de motorische verbetering na DBS bij parkinsonpatiënten met geringe cognitieve stoornissen vergelijkbaar met die van patiënten met een normale cognitie. Hoewel deze bevindingen bij niet-demente parkinsonpatiënten geruststellend zijn, is er op dit moment onvoldoende informatie over de effectiviteit en bijwerkingen van DBS voor motorische responsfluctuaties bij parkinsondementie.
