Elke medische behandeling begint met het stellen van de juiste diagnose. Deze diagnose ligt echter niet altijd meteen voor de hand. Zoals bij de volgende casus:
Een 53-jarige man met een voorgeschiedenis van acute B-cel lymfoblastische leukemie (B-ALL) en een eerdere allogene stamceltransplantatie, meldde zich op de SEH met vermoeidheid, duizeligheid en gedragsveranderingen. Hij was +153 dagen na de transplantatie en gebruikte tacrolimus. De gedragsveranderingen omvatten ontremd gedrag en later apathie en lusteloosheid. Onderzoek toonde ernstige bradyfrenie en een diepe hemolytische anemie (Hb 2,3 mmol/l).
